All industrial accidents are preventable. 

​Chances are that in our lifetime, we may have all been aware of at least one work place incident that may have resulted in a fatality of a family member, colleague or acquaintance.

 

The "Pain-Blame syndrome" kicks in. We often ask "HOW did or could this happen?". Often we look for someone to blame. Responses to these questions provide no guidance to strengthen systems to prevent future incidents.

We generally don't ask the more important question. "WHY did it happen?"

​In the majority of cases, the root cause stems from an absence of adequate systems to prevent the undesirable event, failure to conform to defined standards or follow established procedures.

Not withstanding the pain suffered by affected families, catastrophic incidents may adversely impact a company's license to operate.

The work place especially industrial environments are inherently filled with hazards that pose a threat to the safety and well-being to people, damage to equipment and to the environment. 

 

At Dr.Valve , we take a systematic approach to risk management. 

BELIEVE IT OR NOT: no manager or team leader goes to a work site with a desire to have a safety incident that results in the loss of life!

Generally, safety incidents occur because of:

1. Absence of appropriate systems or processes that  eliminate or mitigate a threat

2. Absence of appropriate standards and requirements for critical equipment and critical tasks

3. Operating procedures developed based on flawed standards 

4. Deviation from or failure to follow proper standards, procedures and/or established processes

At Dr. Valve, we look at risk management from a systems perspective. 

For a defined undesirable safety event, we will build a blueprint using "Bowtie methodology".   

 

These blueprints help visualise critical equipment and critical tasks that either eliminate or mitigate certain threats.

 

These inform the required systems and processes for implementation, assessment and periodic review for continuous improvement.

Escalatory factors and barriers to prevent escalation of an undesirable event are equally important. Are systems, procedures and response plans in place in the event that the worst scenario occurs?

Currently we are focused on risk management in well operations scenarios. The breath and scope of service will be expanded to other industrial processes in the future.

 

The services being offered are leveraged based on the work done by our principal consultant while in full time employment with a major international energy company for the past 22 years and over 4 decades of experience in the oil & gas industry.

During 4 decades, no undesirable event has occurred with a fatality or injury.
